Job description
The Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist (PT) to join our facility. The PT will assess referred patients, establish treatment programs, and educate patients and families under physician direction. Supervising PTAs and ensuring compliant documentation are essential.
Experience with long-term or geriatric care is preferred. The role requires strong patient-centered care and collaboration with all disciplines to deliver excellent rehabilitation services.
