Job description
Habitat Health is seeking an RN Care Manager to deliver personalized care to a panel of PACE participants, conducting comprehensive in-person assessments at centers and in home settings. You will collaborate with the IDT to implement care plans and lead coordinated care across physical, psychosocial, and behavioral domains.
The role requires an accredited nursing degree and an active CA RN license, with 2–4 years of RN experience and 1–2 years in case management.
