Job description
Hartford HealthCare Medical Group is seeking a full-time Nurse Practitioner or Physician Assistant to join the REACH home-based geriatrics program serving patients in the greater Manchester, CT area. The role delivers compassionate, patient-centered care in patients' homes and in assisted living facilities, collaborating with a multidisciplinary team.
Requirements include CT licensure (unrestricted or eligible) and at least two years of post-graduate experience in geriatrics or a related field.
