Job description
TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics focus to improve outcomes for long-term care residents. You will work with a team of clinicians and case managers to deliver coordinated care within nursing homes or assisted living communities enrolled in Medicare Advantage plans.
