Job description
TruHealth is seeking an advanced practice provider (NP or PA) to deliver primary care for seniors in long-term care settings. You will work with a team of providers and case managers to improve outcomes for residents enrolled in Medicare Advantage plans.
You will exercise independent judgment for acute and chronic care, with supervision as appropriate, focusing on quality of life and prevention of hospital visits.
