Nurse Practitioner / Physician Assistant Geriatric Care
Location: Mercer, PA/Grove City, PA
Employment Type: Full-Time
Schedule: Monday Friday Day Shift No Evenings, Weekends, or Holidays
Compensation: $110,000 $120,000 Base Salary + Bonus Opportunity

Position Responsibilities
Provide comprehensive primary care services to geriatric patients in long-term care, skilled nursing, and assisted living communities.
Perform patient assessments, diagnose acute and chronic medical conditions, and develop individualized treatment plans.
Manage chronic diseases while promoting preventative care and wellness.
Collaborate closely with physicians, nursing staff, therapists, and interdisciplinary care teams.
Maintain timely, accurate, and compliant medical documentation.
Communicate effectively with patients, families, and facility staff to ensure continuity of care.
Participate in care planning and quality improvement initiatives designed to enhance patient outcomes.
Qualifications
Required
Licensed Nurse Practitioner or Physician Assistant (or eligible for state licensure)
Excellent communication and interpersonal skills
Compassionate, patient-centered approach to care
Ability to work both independently and collaboratively within a multidisciplinary team
Preferred
Experience in Geriatrics, Primary Care, Internal Medicine, Family Medicine, Long-Term Care, Skilled Nursing, or Assisted Living
Behavioral Health experience is a plus
