Job description
Trinity Health PACE in West Springfield, MA is seeking an experienced Registered Nurse to coordinate and supervise nursing services for participants in the PACE program, delivering care in the day center or in-home settings as needed.
You will work within the IDT to care planning, contribute to QAPI and budget planning, supervise staff, and ensure compliance with regulations while guiding families and coordinating multi-disciplinary care across disciplines.
