Job description
NPAworldwide is seeking a compassionate Home Health Physical Therapist in Greenwich, CT to deliver high-quality patient care in home settings. You’ll evaluate conditions, create personalized plans, and work with a collaborative healthcare team to achieve optimal outcomes.
Responsibilities include educating patients and families, monitoring progress, and adjusting plans as needed. A CT PT license and at least one year of experience are required; home health experience is a plus.
