Terros Health is seeking a Health Center Director for the 51st Avenue Recovery Health Center in Phoenix, AZ. The role leads a multidisciplinary team delivering integrated behavioral health and primary care in an FQHC setting, focusing on life‑long recovery and whole-person care.
The position requires a master’s degree and 5+ years in integrated care or FQHC leadership, with at least 3 years in supervision. RBHA/SMI experience is strongly preferred.
