Job description
The Salvation Army in New York State seeks an Assistant Director of Health Home Care Management to supervise Care Management Supervisors and assist the Director in developing departmental plans compliant with Health Home and NYSDOH regulations.
Full-time position offering generous benefits, pension, health coverage, and 403B with travel across the division. Prior supervisory and health home care management experience is preferred.
