Job description
Community Health Center of Southeast Kansas, Inc. (CHC/SEK) is seeking a Health Promotion Nurse to outreach patients recently discharged and coordinate care within a multidisciplinary team. You will build rapport, address barriers, develop patient care plans, and perform medication reconciliations to improve health outcomes.
You will provide chronic disease education, leverage EMR and telehealth tools, and collaborate with providers to support transitions of care and follow-up care planning.
