Job description
Sagility is seeking Seasonal Work-From-Home Care Coaches to support health plan members through proactive outreach and education. In this role, you’ll conduct phone-based assessments, explain benefits, and help members navigate care programs to improve engagement and health outcomes. Using Sagility’s technology platforms, you’ll document interactions, coordinate appointments or referrals, and follow standardized workflows. You’ll work in a performance-driven, inclusive team environment focused on measurable results, service quality, and compliance with HIPAA and organizational policies.
