Job description
Sagility is hiring Seasonal Work-from-Home Care Coaches to support health plan members by phone and digital channels. In this role, you will conduct outbound and inbound calls, assess member needs, and provide education on benefits, preventive care, and condition management. You will schedule appointments, coordinate with providers, and document all interactions in care management tools while adhering to HIPAA and quality standards. Working remotely, you’ll collaborate with a supportive team, receive thorough training, and help drive better healthcare experiences and outcomes for members.
