Job description
The Outreach Specialist is responsible for enrollment of prospective participants into the PACE program.
Responsibilities include directing and coordinating all marketing, outreach, and community engagement activities, managing intake/enrollment processes, and reporting on team performance with a focus on efficiency and conversion. Collaboration with referral sources and DHCS is required to maintain compliant enrollment workflows.
