Job description
Pc CFL is seeking a Facility Navigator to act as the main liaison between the Skilled Nursing Facility and Home Physicians Group. You will support staff, document patient information, and coordinate with schedulers and providers.
The role requires CMA/MA certification or equivalent experience, familiarity with EMR systems, and strong communication and organizational skills. On-site position with competitive benefits is offered by Home Physicians Group.
