At least 32 patient contact hours

Radiation Oncology department
Robust Oncology supportive services
24‑bay OP Oncology infusion department with on‑site pharmacist
Requirements
Active SC license or willingness to obtain one
BC/BE (if BE, within 5 years of eligibility)
DEA/CSR (must be obtained prior to start)
NPDB Self‑query report (within 30 days active)
