Job description
Indiana Hemophiliaand Thrombosis Center in Indianapolis seeks a Pharmacy Nurse Care Coordinator to provide advanced clinical coordination for patients with bleeding disorders. You will support individualized treatment plans, manage complex cases, and collaborate with IHI physicians and multidisciplinary teams.
The role includes adherence monitoring, insurance navigation, home visits, scheduling, and education on bleed management, with on-site presence at our center in Indianapolis.
