nSalary Range: Position starts at $18.65/hour
nShift Differential: +$1.50/hour for 3rd shift

nLutheran’s Hultquist Place—an Assisted Living Program (ALP) operating in coordination with Licensed Home Care Services Agency (LHCSA) standards—is seeking a compassionate and dependable Certified Home Health Aide (HHA) to join our overnight team.
nOur HHAs play a vital role in supporting residents’ independence, dignity, and safety—especially during overnight hours when attentiveness and responsiveness matter most.
nPosition Summary:
nUnder the supervision of the Director of Patient Services (DPS) and licensed nursing staff, the HHA provides personal care and supportive services in accordance with each resident’s individualized care plan and applicable ALP/LHCSA regulations. This role includes assisting with self-administration of medications (following facility training and protocols), overnight monitoring, and timely communication of any changes in resident condition.
nQualifications:
nnnHigh School Diploma or equivalent required
n
nnValid New York State HHA Certification required (or equivalent approved training/exam)
n
nnOR Certified Nursing Assistants (CNAs) willing to complete HHA conversion testing are encouraged to apply
n
nnPrior experience in assisted living, home care, or long-term care preferred
n
nnMedication assistance experience preferred (training provided)
n
nnBasic computer skills for EMR documentation
n
n nKey Responsibilities: Resident Care & Support
nnnProvide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, toileting, grooming, and mobility
n
nnPromote resident independence, dignity, and choice in all interactions
n
nnConduct routine overnight rounds to ensure resident safety, comfort, and well-being
n
nnRespond promptly to call lights and resident needs
n
n nMedication Assistance
nnnAssist residents with self-administration of medications in accordance with ALP guidelines and facility policies
n
nnFollow all medication assistance protocols, including proper documentation and reporting
n
nnImmediately report any medication concerns, refusals, or discrepancies to licensed nursing staff
n
nnDo not interpret physician orders or independently administer medications outside of scope
n
n nMonitoring & Reporting
nnnObserve, document, and report changes in resident condition, behavior, or needs to the Charge Nurse/DPS
n
nnAccurately complete documentation in the electronic medical record (EMR) in a timely manner
n
nnReport incidents, safety concerns, or unusual findings per policy
n
n nCare Coordination & Teamwork
nnnCommunicate effectively with nursing staff, leadership, and interdisciplinary team members
n
nnSupport a safe and respectful environment for residents, families, and coworkers
n
nnParticipate in shift-to-shift communication to ensure continuity of care
n
n nEnvironment & Support Services
nnnProvide light housekeeping and resident support services as needed
n
nnAssist with meal-related needs, hydration, and nourishment monitoring (as applicable overnight)
n
nnFollow all procedures related to admissions, discharges, and transfers
n
