Medical Assistant Care CoordinatorWe are seeking a dedicated and compassionate Medical Assistant. This position is ONSITE 5 days a week and 100% Phone based.Pay Rate:Entry Level: $20 an hourExperienced (2 years minimum): $22.50 an hourResponsibilities:Conduct monthly CCM and RPM patient outreach calls.Review medications, treatment plans, and patient concerns.Monitor blood pressure readings and respond to RPM alerts.Coordinate care between patients, providers, pharmacies, and medical equipment vendors.Assist with obtaining and facilitating physician orders for durable medical equipment.Document patient interactions and maintain compliance within the CCM/RPM platform.Ensure Medicare-required patient engagement time is achieved and documented.Educate patients and provide support regarding chronic health conditions.Identify barriers to care and escalate concerns to providers when necessary.Essential Skills:Medical Assistant DiplomaStrong interpersonal and communication skillsWork Environment: This position is office-based, working onsite Monday through Friday from 9:00 AM to 5:00 PM in Fresh Meadows, Queens, NY.This is a Permanent position based out of Fresh Meadows, NY.The pay range for this position is $ - $ /yr.
