Job description
SUNY Upstate Medical University is seeking a highly motivated Retention and Adherence LPN Specialist to support patients in Inclusive Health Services and the Pediatric Infectious Disease Clinic's grant-funded program.
The role provides linkage to and retention in medical care with intensive case management for patients new to ART or not virally suppressed, focusing on overcoming barriers and improving health outcomes. Strong teamwork required and adherence to institutional policies.
