Job description
Erie Family Health Centers in Chicago seeks a Team Leader for the Lending Hands for Life (LHL) HIV program. The role oversees medical and non-medical case management for HIV+ clients, manages intakes and EMR caseloads, and coordinates service referrals to ensure holistic care.
