Job description
Hartford HealthCare seeks a Nurse Practitioner or Physician Assistant for the REACH program, delivering geriatric care at patients’ homes and in assisted living settings in the greater Manchester, CT area. The role focuses on comprehensive home-based assessments, care planning, and multidisciplinary collaboration.
The ideal candidate holds CT licensure, is trained as an APRN (Family/Adult/Geri) or PA, and has at least two years of post-graduate geriatric experience.
