Job description
Gentiva's SouthernCare New Beacon Hospice seeks a compassionate Hospice RN Case Manager to serve Madison & Limestone Counties. You will be the primary nurse guiding patients and families, delivering hands-on care in home settings and coordinating with the IDG team to implement the Plan of Care.
You will perform assessments, document care, and maintain compliance with infection control and regulatory standards while supporting on-call duties as needed.
