Job description
Matrix Medical Network seeks a Nurse Practitioner to perform in-home health assessments for adults and seniors. You will review medical history, adherence, and social factors to provide a comprehensive view of health during home visits.
Collaborating with physicians, case managers, and families, you will help close care gaps using the Matrix Comprehensive Health Assessment and coordinate follow-up across settings.
