nJob Description
nnnAccept and manage patients within your designated geographical area of coverage
n
nnPlan, prepare, and implement customized therapy programs designed to restore overall function and promote independence in the home setting
n
nnDevelop and maintain individualized care plans/goals in collaboration with physicians, patients, and their families
n
nnEnsure timely and accurate documentation of all patient visits and therapy services
n
nnEffectively communicate discharge planning with patients, families, and interdisciplinary team members
n
nnMaintain compliance with all clinical and documentation standards
n
n nQualifications
nnnPhysical Therapist Program Graduate.
n
nnTN Physical Therapist License or eligible.
n
nnLiability insurance
n
nnHome health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
n
nnCertifications Required: CPR/First Aid
