Job description
Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to join our interdisciplinary team, delivering advanced palliative care in patients' homes and guiding families through treatment decisions.
You will collaborate with the Palliative Medical Director, perform comprehensive assessments, and develop individualized care plans while managing medications and documenting care in compliance with standards.
