Job description
Gentiva, Empatia Palliative Care, seeks a compassionate Palliative Care Nurse Practitioner to extend expert, home-based palliative care. You will assess, plan, and manage complex cases in collaboration with a care team, guiding patients and families through treatment decisions and symptom management.
You will work autonomously with physicians, maintain compliance, and contribute to quality outcomes while supporting families in meaningful ways.
