Job description
Empatia seeks a compassionate Palliative Care Nurse Practitioner to deliver advanced care in patients' homes, collaborating with a Palliative Medical Director to optimize quality of life and manage complex medical needs.
You will assess physical, emotional, and psychosocial factors, develop individualized care plans, and coordinate with families and the care team. A strong background in palliative care and Part B billing is preferred.
