Job description
Optum NY/NJ seeks a Nurse Practitioner or Physician Assistant to join our team in Orange County, NY. The role includes providing care in patients' home settings with 5-8 visits per day, plus potential video visits for established patients.
As part of the Optum Care Delivery team, you will collaborate with PCPs to develop care plans, maintain medical records, and support preventive care in a patient-centered, technology-driven environment.
