OptumHome & Community Care is seeking a Nurse Practitioner or Physician Assistant to provide comprehensive primary care in patients' homes across Orange County, NY.
This home-based role focuses on longitudinal, relationship-driven care for medically complex patients, working with an interdisciplinary team to improve outcomes while offering meaningful, value-based care outside of traditional clinics. Independent practice with strong clinical support is available.
