Housecall Providers is seeking a Nurse Practitioner (NP) for Primary Care in the Portland metro area. This 2-day-per-week role provides in-home care and telehealth visits, focusing on chronic disease management and palliative care goals with patients in home settings.
The position emphasizes assessment, on-boarding, and coordination of care with patient’s PCPs. Requires Oregon NP license, DEA and CPR, with strong communication and teamwork skills for delivering compassionate, high-quality care.
