Job description
The U.S. Department of Veterans Affairs seeks a Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) to provide evidence-based nursing care to HBPC patients and supervise LVNs/ CNAs as appropriate.
The RN will manage all elements of the nursing process, perform home visits, ensure care coordination, education, and participate in quality improvement with the interdisciplinary team. This role requires a valid state license, nursing degree, and compliance with VA qualification
