Job description
WellBe Senior Medical is seeking a Part-time Physician Assistant (PA) to deliver high-quality, home-based primary care for seniors in Rochester, New York. In this role, you will conduct thorough visits, perform exams, and coordinate care with an interdisciplinary team to improve outcomes and reduce hospital visits.
You’ll support chronic disease management with personalized care plans tailored to each patient’s needs.
