Job description
The U.S. Department of Veterans Affairs’ Joplin CBOC is seeking a Home Based Primary Care Staff RN to provide direct Veteran care, care coordination, and leadership in HBPC. The RN will perform case Management, assessments, and care planning in the Veterans’ homes.
Responsibilities include home visits, telehealth, interdisciplinary collaboration, and supervising related nursing staff; a comprehensive federal benefits package accompanies the role.
