Job description
NYU Langone Hospital—Long Island is seeking a Home Care Intake Nurse - Per Diem. The successful candidate will complete comprehensive assessments of patients referred to the agency, plan admission, determine eligibility, and coordinate approvals.
The role requires an Associate's Degree in Nursing and NYS RN license, 1–2 years in acute care or home health, strong communication, critical thinking, and computer skills, and a commitment to delivering high‑quality care in home settings.
