n nWhat you'll need:
nnnActive LPN or RN License issued by NYS
n
nnMinimum of 6 months experience in a home care setting.
n
nnCurrent CPR/BLS Certification
n
nnEmpathy, Reliability , and a commitment to high-quality care
n
n nWhat you'll do :
nnnPerform 1:1 patient care duties and treatments as per care plan.
n
nnProvide health promotion, counseling, and education.
n
nnAdminister medications and other personalized interventions.
n
nnCoordinate care, in collaboration with a wide array of health care professionals.
n
nnDocument nursing notes regarding patient care daily.
n
nnMust be able to lift 50lbs.
n
nnMust be able to stand/work for extended periods
n
nnAdditional responsibilities as required.
