We are looking for a Registered Nurse interested in providing home care and case management.

RN Responsibilities:
Provide thoughtful care to clientsbased on personalized Plan of Care
Provide in home assessments
Monitor and track changes in the clientsconditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
Proactively updates client'sfamily on plan of care, health changes, and concerns
RN Requirements
Must have at least 1 year experience
Current unrestricted RN license in Utah
Certified CPRCertification
Negative TB skin test or chest x-ray
Must be able to pass a drug test and a background check
Licensed driver with valid automobile insurance
Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift 50 pounds
