Job description
Provides home-based occupational therapy evaluations and treatment, develops and updates patient care plans, and supports patients’ functional abilities, rehabilitation, and safety. Works with patients, families, physicians, and interdisciplinary care teams, recommends adaptive equipment and home modifications, and documents progress and clinical services in accordance with applicable standards.
