Job description
Allina Health is seeking a Home Care Physical Therapist to provide therapy and education to clients across an assigned geographic region. You will assess, set goals, and progress plans of care while coordinating with families, caregivers and other disciplines.
Responsibilities include case management, referrals, and ensuring high-quality, patient-centered care. A valid MN-licensed PT and BLS are required; prior home care experience is preferred.
