Job Description
Home Care Registered Nurse (RN)
We are seeking a dedicated and compassionate Home Care Registered Nurse (RN) to join our team in delivering exceptional home health and hospice care. This is a full-time position with a Monday through Friday schedule, which will include weekend rotation approximately every fourth week.
Job Overview
The Home Care RN is responsible for providing individualized, goal-oriented nursing care to patients in their homes, following the nursing process according to physician orders. This role will also serve as the Case Manager for cases where the RN is the lead discipline.
Key Responsibilities
Provide comprehensive nursing assessments and develop care plans based on individual patient needs.
Deliver direct patient care, including administering medications and treatments as prescribed.
Monitor patient progress, adjusting care plans as necessary to ensure optimal outcomes.
Educate patients and families on health promotion and disease prevention strategies.
Coordinate with other healthcare professionals to ensure holistic care for patients.
Qualifications Must possess a valid Registered Nurse (RN) licensure.
Minimum of 1 year of nursing experience, preferably in home health or hospice care.
Current Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) certification.
Strong clinical and critical thinking skills.
Excellent communication and interpersonal abilities to interact with patients and their families.
