Job description
Angels Care Hospice in the United States is seeking a dedicated Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will assess patient needs, interact with other medical disciplines, and develop the plan of care for in-home services.
You will perform admission visits, determine eligibility for home care, and ensure high-quality care while supporting patients and families.
