Job description
Granite VNA in Concord, NH is seeking a Registered Nurse (RN) for Home Care, working Friday through Sunday (8 AM–6 PM). This full-time role involves delivering one-on-one home health care, coordinating with the care team, and supporting patients in their homes across Concord and surrounding towns.
As a Home Health RN, you will assess, plan, implement, and evaluate patient care while supervising LPNs and CNAs.
