nPosition Purpose:
nThe Palliative Care RN is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation.
nWhat You Will Do:
nnnCollaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation
n
nnMotivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy
n
nnProvides statistical/budgetary data to appropriate Palliative care program
n
nnWorks collaboratively with all departments to ensure execution of quality delivery of services
n
n nMinimum Qualifications:
nnnGraduate of an approved nursing education program.
n
nnCurrent Registered Nurse licensure in the state of practice.
n
nnCPR certification
n
nnTwo years acute background and Hospice/Palliative Care certification preferred.
n
nnHome care experience, strong clinical knowledge base, Hospice experience, computer skills, strong interpersonal and organizational skills,
n
nnPalliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills
n
nnMust be able to work independently and on a team
n
nnAbility to perform diversified duties with time limitations
n
nnAbility to develop, coordinate and evaluate programs in Palliative care and end-of-life care
n
