Overview
5 x 8 hour shifts (9:30 AM to 5:30 PM) - rotating weekends and holidays.
Great healthcare benefits, and education reimbursement of $5,250 annually.
Additional compensation for visits over your caseload.
This position will travel between Catholic Health's Skilled Nursing Facilities.
Job Details
The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.
Our Home Care Transition Nurses
Establish and maintain professional relationships with key physician groups and referral sources.
Act as liaison to referral sources on behalf of Catholic Home Care.
Identify potential market growth opportunities and program expansion.
Serve as a customer service representative and act in a professional and supportive manner at all times.
Requirements
Registered Nurse with current licensure in New York State.
Two or more years of home care experience with patient assessments.
Customer service and marketing skills.
Salary Range
USD $90,000.00 - USD $105,000.00 per year.
The range is a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses, incentives, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
