Job description
Lehigh Valley Health Network is seeking a dedicated RN for Home Care & Hospice triage nursing. You will provide clinical support by triaging calls, entering data, and coordinating patient care with scheduling and ongoing communication.
Ideal candidates have nursing degrees (Associates or Bachelors) and relevant experience in home health, with strong clinical judgement and excellent communication skills. This full-time day role covers Bucks, Montgomery, and Chester counties.
