Job description
Fresenius Medical Care is seeking an entry‑level Home Therapies RN for the Transitional Care Unit (TCU) in Illinois. You will coordinate patient care, provide education for home dialysis, and work with the End Stage Kidney Disease team to improve patient outcomes and facility operations.
Requires RN licensure, nursing degree, and CPR/BLS; nephrology or home dialysis experience preferred. The role involves on‑site patient care, teaching, and collaboration across disciplines.
