Job description
Canyon Home Care and Hospice is seeking a Home Health Admission Registered Nurse (RN) to perform Start of Care assessments and admission visits in patients' homes. You will evaluate clinical needs, homebound status, and eligibility for services, then develop an individualized plan of care in collaboration with the patient, family, physician, and interdisciplinary team.
The role requires RN licensure, CPR/BLS, and strong assessment and communication skills.
