nJob Summary As a Home Health Aide, you will provide care and support to multiple members residing in the same group home, following individualized care plans. This role is ideal for someone with a big heart who values building genuine relationships while providing attentive, personalized care. You should be organized, detail-oriented, and committed to creating a safe, healthy, and supportive environment that meets each member's needs. Strong communication skills are important. Prior Home Health experience is a plus.
nResponsibilities
nAssist members with ADLs, including bathing, dressing, toileting, grooming, and safe mobility
nSupport meal preparation and encourage healthy eating routines
nHelp members transfer in and out of beds, baths, or wheelchairs
nMaintain a clean, safe living environment, including changing bed linens, doing laundry, and light housekeeping
nKeep accurate records of care, observations, and any changes in condition
nCommunicate updates or concerns to the supervisor
nProvide companionship and social engagement to help members remain active and connected
nQualifications
nGraduation from an accredited Home Health Aide program
nHigh School Diploma or GED
nOne year of prior professional experience preferred
nValid driver's license
