nnnAssisting clients with daily living activities, including bathing, grooming, dressing and feeding.
n
nnAssisting patients with mobility and transfers.
n
nnCompleting necessary documentation accurately and promptly.
n
nnCommunicating effectively with clients, families and other healthcare professionals regarding client needs and concerns.
n
nnFollowing established care plans and contribute to the development of personalized care strategies.

n nJob Requirements
nnnHigh School Diploma or GED
n
nnCNA-Completion of 75, 120 or 150 hour Certified Nursing Assistant Pgm/Must be in State C.N.A registry
n
nnHHA-Successful completion of a competency evaluating program
n
nnCNA-One year of Certified Nursing Aide-Home Health Hospice
n
nnHHA-One year home health aide, patient tech or other related experience and/or passes CHI Health at Home's competency
n
