Home Health Aide/Certified Nursing Assistant (HHA/CNA) - Join Our Synergy Family!

Responsibilities:
Provide personal care and support services to patients, including assistance with daily living activities, such as bathing, dressing, and grooming
Assist with mobility and transfers, including use of wheelchairs, walkers, and canes
Administer basic care, such as taking vital signs, feeding, and toileting
Perform light housekeeping and laundry tasks to maintain a clean and safe environment
Assist with medication reminders and administration (as permitted by state law)
Collaborate with a multidisciplinary healthcare team to develop and implement individualized care plans
Educate patients and families on self-care techniques and disease management
